How Do I Get Started?

  1. Research bariatric surgery and determine if it is something that you really want to do.  Obesity surgery is not an "easy way out" or a "quick fix".  It is a major endeavor that helps most people but has resulted in death and unpleasant experiences for a few. 

  2. Find out what your body mass index (BMI) is and determine if you are a candidate for surgery.  You can do this by reviewing the "Do I Qualify" section of this site.

  3. If you are a candidate, you may not be sure which procedure is best for you.  This is normal.  You an learn more about your options at The American Society for Metabolic and Bariatric Surgery.  All of your questions/concerns will be answered at your initial consultation.  Together, you and your surgeon can reach a decision about which procedure is right for you.

  4. Contact Kellie, our bariatric secretary, at (330) 344-1950 with your insurance information.  Kellie will be happy to check with your insurance company to see if you have benefits for bariatric surgery and to see which procedures are covered.  Some insurance companies have limits on bariatric coverage.  If so, full prepayment is required prior to scheduling surgery.

  5. Down load and print the medical history form below or call our office to have one mailed to you.  Fill our the form and mail it to the address on the last page of the form.

  6. After we have reviewed your medical history form we will contact you regarding an appointment.  If you are not contacted within 7 - 10 days after mailing your medical history form, please call our office at (330) 344-1100.   

    Please note that if your insurance company does not cover the initial office consultation, you will be charged $300 for the visit.  This is rare.  Once you have gone through the insurance approval process (6 months for most plans) and preoperative studies (usually 1 month), a $1,400 conditionally-refundable program fee will be required when your surgery is scheduled.  This is described in the Program Fee Disclosure and Contract form which you can download and review below.  You will be required to sign this form before surgery can be scheduled.
